Welcome to Final Forms, our online tool for submitting and maintaining all needed information for student-athletes and cheerleaders in grades 7-12, and also ZHS band!
Parents or legal guardians first will create their accounts on Final Forms and then complete all needed information. Need to make updates? Just log back in and make changes as necessary. All of this will negate the distribution and collection of numerous paper forms!
The student, after they have been added to our system by their parents or legal guardians and all information has been entered, will then be able to log in to complete their information. (NOTE: Expiration dates of the official OHSAA pre-participation physicals [not required for band members] will be entered by our Final Forms administrators.)
