Sports Physicals

 The Ohio High School Athletic Association mandates that all student-athletes have a current physical on file with our Athletics Department prior to any participation, including practices. Parents may elect to have a family physician perform the physical, but it must be completed using the official OHSAA Pre-Participation Physical Form (available for download at right). Physicals must be renewed on an annual basis.
  
Orthopaedic Associates of Zanesville and Sports Medicine Zanesville are again offering their FREE sports physicals for student-athletes entering grades 7-12. OAZ/SMZ "staggers" the start times for each school district — Zanesville student-athletes begin at 8 a.m. — but those who can not make that time may come at any other time during the event. Appointments are not required. For more information, call 740-588-1089.

Please follow these very important guidelines:
  1. Be sure you have the correct OHSAA physical form (dated 2018-2019; a link to the file is at the top of the right-hand column on this page.
  2. Be sure the form is completed (other than the physician's page) and it is signed in all places as required for parents/guardians AND the student-athlete.
  3. If the student-athlete wears glasses, be sure to bring them to the physicals.
  4. Be sure to mark the school year on the form as the one the student-athlete will be in beginning the 2018-2019 school year.

To print a physical form direct from Final Forms:
  • Log into Final Forms.
  • Click the last form on the list (OHSAA PPE Physical [for physicians]).
  • Print the form.
  • Sign the form and take it with you to the physician(s) office.
