Kindergarten registration is coming!
 Here is what you need to know:

REQUIREMENTS

Please familiarize yourself
with the information below
before registering your child
for Kindergarten.
BE PREPARED!

ELIGIBILITY: Prospective students must be FIVE (5) YEARS OLD on or before August 1st prior to the school year he/she begins Kindergarten.
 
REGISTRATION: Your child will be registered online.

ADVANCEMENT: All students must successfully complete Kindergarten before enrolling in the first grade.

DOCUMENTATION: Each prospective Kindergarten student must have the following paperwork brought with them to registration – a copy of the child's REGISTERED birth certificate (obtained from the Health Department in the county of birth), immunization records (see below for details), proof of residency (current utility bill, pay stub, lease agreement, etc.), and custody papers (if applicable).

IMMUNIZATIONS: Ohio law requires that ALL pupils admitted for initial entry to school present written evidence that they have received, or are in the process of receiving, the following immunizations – 
 • Poliomyelitis (OPV)
 • Diphtheria-pertussis-tetanus (DTAP)
 • Measles-mumps-Rubella (MMR)
 • Hepatitis-B
 • Varicella (chicken pox)
Children are required to have 5 DTAP, 4 Polio, 2 MMR, 3 Hepatitis-B and 2 Vericella vaccinations BEFORE school starts.
