Here is what you need to know:

Wednesday, March 20 2 to 7 p.m.
Thursday, March 21 10 a.m. to 3 p.m.

Requirements

Please familiarize yourself
with the information below
before registering your child
for Kindergarten.

COME PREPARED!

ELIGIBILITY: Prospective students must be FIVE (5) YEARS OLD on or before August 1st prior to the school year he/she begins Kindergarten.
 
REGISTRATION: Your child will be registered at the elementary building where your child will attend according to your address of residency.

ADVANCEMENT: All students must successfully complete Kindergarten before enrolling in the first grade.

DOCUMENTATION: Each prospective Kindergarten student must have the following paperwork brought with them to registration – a copy of the child's REGISTERED birth certificate (obtained from the Health Department in the county of birth), the child's Social Security card, immunization records (see below for details), proof of residency (current utility bill, pay stub, lease agreement, etc.), and custody papers (if applicable).

IMMUNIZATIONS: Ohio law requires that ALL pupils admitted for initial entry to school present written evidence that they have received, or are in the process of receiving, the following immunizations –
• Poliomyelitis (OPV)
• Diphtheria-pertussis-tetanus (DTAP)
• Measles-mumps-Rubella (MMR)
• Hepatitis-B
• Varicella (chicken pox)
Children are required to have 5 DTAP, 4 Polio, 2 MMR, 3 Hepatitis-B and 2 Vericella vaccinations BEFORE school starts.
